Commitments and Responsibilities
Our clients matter. It is our aim to offer quality care in service of each client’s well-being.
Services Offered

Tele-therapy services are available to clients who live in Maryland
Scheduling and Canceling Appointments

I personally arrange scheduling to be most responsive to the needs of clients

When we agree on a time slot, no other client can be scheduled during that allotted time

Contact me by email (youmattercounseling@gmail.com) in order to schedule or change appointments

Clients are asked to provide at least 24 hours’ notice for cancellations or rescheduling

Clients are expected to pay for missed sessions due to cancellations made less than 24 hours in advance

If I fail to provide 24 hours’ notice of cancellation, the next session is at no expense to the client
Payment and Insurance

I am an out of network provider for all insurances

Payment is due at the time of service unless otherwise arranged

I accept various forms of payment, including all credit cards (Visa, Mastercard, American Express, etc.)

Clients are responsible for understanding their insurance coverage and benefits
Confidentiality Practices

All client information is kept confidential in accordance with HIPAA regulations.

Therapist will not disclose any information without the client’s consent, except in situations where there is a risk of harm to self or others, or as required by law
Grievance Policy

If a client has concerns about their therapy experience, they are encouraged to discuss these with the therapist, Symone Colquitt, LCMFT

Refunds for services will be provided after grievances have been reviewed by client and the therapist

Any concerns can be forwarded to youmattercounseling@gmail.com . Please allow 5 to 10 business days for a response
Amendments

This fulfillment policy is subject to change. Clients will be notified of any significant changes in a timely manner
